Trinity Advocacy Group, a based care management and advocacy firm focusing on seniors, adult disabled, and their families, providing care management, professional guardianship and fiduciary services, and elder mediation seeks a part time Assistant Care Manager, who is Master’s prepared in the human services field, such as nursing, social work, gerontology, and/or extensive experience in care management role with seniors and/or the disabled. Also has superior people-skills, ability to analyze care risks and identify reasonable solutions, skillful at providing compassionate client direction, encouragement, and support while maintaining professional, appropriate boundaries. Team player in a collaborative environment working on a variety of interesting cases, while also possessing the ability to work independently. Possibility of full time available.
