DuPage County has a Clinical Case Manager position within Kenneth Moy DuPage Care Center. This position will provide professional clinical case management to patients and clients coordinating services, treatment status, discharge plans and communicating benefits and financial information. Below lists some responsibilities of the position:
Reviews, assesses and documents benefit, medical, social and financial information in an accurate and appropriate manner.
Establishes contacts with family, payer and other internal and external customers.
Develops and coordinates safe, effective, realistic and timely discharge plans in coordination with patient, family and team and prepares discharge papers.
Conducts Minimal Data Set (MDS) interviews and completes assigned sections of MDS 3.0 including assigned Care Area Assessment (CAA).
Completes initial assessment, social history, and discharge assessment.
Schedules and coordinates care plan meetings.
Maintains and updates discharge calendar for interdisciplinary coordination of services.
Assists new patients/families with the completion of admission paperwork.
Develops and maintains current documentation in the medical records regarding status of discharge plans, case management authorizations, utilization review and family issues related to discharge, mood, behavior issues, and progress notes.

To get this job, you typically need a Minimum of 3 years’ experience working with electrical, hydraulics, programming, and mechanical PLCs is required.
We pay $2.00 per hour shift differential for all time worked on 3rd shift. *Rate of pay depends on one’s experience, education, and skills.

All Technicians are sent to College of DuPage for classes. Technicians are scheduled for a series of six mechanical and six electrical classes. Classes are during work hours, so Technicians are paid for their time in class. We adjust their hours to spend half their shift on the floor and the balance in the class. Jel Sert pays for this training which includes classroom and lab work. All Technicians are involved in this training despite their experience level.
