The DuPage County Health Department is looking for a new team member to join their intensive home visiting services team as a Home Visitor/Case Manager for the Healthy Families Program.
As a home visitor/case manager, you will be responsible for building and sustaining a relationship with a caseload of diverse families who are pregnant or parents of newborns through the child’s earliest years. This position requires travel throughout DuPage County. Dual language candidates are encouraged to apply.
Salary is $22.77 per hour. - Works with and provides services to young children and their families, by providing weekly home visits at the start of services.
- Outreaches families and builds and establishes trusting, long term (3-5 year) relationships with families.
- Manages a client caseload consistent with department productivity standards.
- Meets with clients in their homes and in the community throughout DuPage County. Also maintains communication with clients via phone, text, and video visits.
- Assesses family needs. Â Conducts assessments and screenings including parent interview about the family’s story and screenings for developmental functioning, depression, substance use, and family violence. Connects families with appropriate resources.
- Utilizes reflective practices to support and strengthen parent-child interaction and overall family functioning. Â Helps families develop and implement practical approaches to real-life situations.
- Participates in weekly individual reflective supervision and monthly group reflective consultation.
- Completes and maintains required documentation within established time frames.
- Participates in emergency response activities as assigned.
- Maintains required training, licensure, and/or certification.
- Maintains confidentiality of privileged information and adheres to patient privacy laws.
- Demonstrates sensitivity and understanding of other ethnic groups and cultures.
- Adheres to department guidelines for attendance and punctuality.
- Performs community outreach activities.
